Responsibilities:
- Conduct comprehensive psychosocial assessments and provide counseling services to patients and their families
- Offer case management and coordinate services to optimize patient functioning
- Refer patients to community services and work with the interdisciplinary team for care planning
- Develop resources to meet patient needs and participate in orientation, education, and rehabilitation programs
- Advocate for patients within the renal setting and participate in professional social work organizations

Requirements:
2+ years of experience as a Licensed Social Worker Master's degree in social work LMSW or LCSW license in the state of New Mexico Compensation:
- Above average hourly rate, based on experience
